We want you to understand clearly how we work so you can get the very best of care, consistently. Our policies are in place to help us provide excellent care to you each time you visit.
|Proof of Insurance||Please present your insurance card(s) at each visit.|
Same-day appointments are available for acute illnesses and injuries. Routine checkups and physicals are normally scheduled within one to three weeks, depending on the specific needs of the patient.
We ask that you arrive 10-15 minutes prior to your scheduled appointment time so we can update your information without delaying your visit with your provider.
|Cancellations||If you need to cancel an appointment, please do so as early as possible so that we can provide another patient an opportunity to be seen.|
|Payment||Copayments are collected at the time of your visit. We accept cash, checks, Visa and MasterCard.|
|Privacy||Please review the HIPAA information in your chart regularly and notify us if you wish to change any of your contact information. We cannot provide health information to anyone who is not listed on your HIPAA page.|
